Slovakia Tourist Visa

Visa Application Form – signed by the applicant (mandatory)
Photograph – recent (≤6 months), light background, 3.4 cm X 4.5 cm
Passport – valid at least 3 months after intended stay, issued within last 10 years, with 2 blank pages; include copy of ID page and previous Schengen visas (if any)
Emirates ID and residence visa
Minors (under 18) traveling alone – parental/legal guardian authorization (original or legalized) and copies of parents’/guardians’ passports

Bank statements – last 3 months, the original statement should be signed and stamped and showing regular income.
Sponsor documents: Sponsor’s passport and visa copy, salary certificate and current bank statement and relationship proof.
Employees: NOC (No Objection Certificate) on official letterhead with employer’s contact info, employee details, salary, position, leave approval
Self-employed: valid trade license

Full travel itinerary
Round-trip flight reservation with valid code or ticket number (if already paid)
Proof of accommodation
Travel medical insurance – valid for the entire stay, minimum coverage €30,000 (medical reimbursements, emergency evacuation, repatriation)
Cover letter explaining the purpose of travel

Business Travelers: Invitation letter from a Slovak company (in Slovak language) or an authority to attend a meeting, conference stating contact details of the host (mobile phone and email. Provide copy of host Slovak ID.
In case of invitation (if visiting friends/family) : Required invitation from host, copy of inviter's passport and ID and specifies the relation with inviter.
Any additional documents that strengthen the application (e.g., property ownership, return tickets, previous Schengen visa history)
Marriage certificate (if spouse is travelling)
Birth certificate (if child is travelling)
