Visa Application Form – signed by the applicant (mandatory)
Photograph – recent (≤6 months), light background, 3.4 cm X 4.5 cm
Passport – valid at least 3 months after intended stay, issued within last 10 years, with 2 blank pages; include copy of ID page and previous Schengen visas (if any)
Emirates ID and residence visa
Minors (under 18) traveling alone – parental/legal guardian authorization (original or legalized) and copies of parents’/guardians’ passports
Copy of all previous Schengen Visa with Entry and Exit Stamp (if any)

Bank statements – last 3 months, original and salary credited statements.
Sponsor’s documents: If your trip is sponsored, you will generally need to submit salary slips, bank statements and the sponsor's trade license and relationship proof.
Employees: NOC (No Objection Certificate) from the company.
Self-employed: valid trade license

Full travel itinerary
Round-trip flight reservation with valid code or ticket number (if already paid)
Proof of accommodation covering the full stay: Hotel reservation, Rental or ownership documents in CZ, Private accommodation confirmation (host covering costs) or Invitation from a company (company covering costs)
Travel medical insurance – valid for the entire stay, minimum coverage €30,000 (medical reimbursements, emergency evacuation, repatriation)

Business Travelers: Invitation letters from company/authority for meetings, conferences, trade fairs, or other work purposes
Students: NOC from the school / university : to indicate ongoing registration and time of absence.
Any additional documents that strengthen the application (e.g., property ownership, return tickets, previous Schengen visa history)
Cover letter
Marriage certificate (if spouse is travelling)
Birth certificate (if child is travelling)
